How to get started with telehealth:
- Fill out our telehealth signature pages
- Go to Patient Forms on our website
- Read and sign the Telehealth Consent
- Read and sign the first page of Telehealth Scheduling Forms
- Send/ Return both signature forms to our office
- Make sure you have:
- Good internet connectivity
- Your computer/ phone has all the system requirements (see our form on this)
- A quite place to meet with us without distraction
- A photo ID
- A patient portal set up with our clinic
- Call to schedule your appointment
- Our staff will send you a portal message with the video link
- Log in 10 minutes before your scheduled appointment
- Paste this link into your internet browser and hit enter
- Make sure your video and microphone are working before the visits starts
- At the visit
- Your provider will ask to see you photo ID before the visit starts
- Be honest and have a great experience with your provider
- After your visit
- Contact our office to schedule a follow up
- Follow any recommendations provided to you
