Advance directive

An advance directive is a legal form you fill out. It says what kinds of medical care you want if you aren't able to make decisions for yourself. It tells your family and your doctor what to do if you're badly hurt or have a serious illness that keeps you from saying what you want.

There are two main types of advance directives:

  • Living will. This is also called a declaration.
  • Medical power of attorney. This is also called a durable power of attorney for health care.

Welcome Dr. Andrea Habel

On August 24th, 2021, Andrea (Annie) Habel, MD joined our practice! Annie spent time with us and our patients last August as a Resident and we are happy to announce that she has decided to start her career with us! She graduated from the University of Washington School of Medicine (WWAMI Program) and completed her residency at the University of Colorado Swedish Family Medicine in Denver. She is also a current active member of the Wyoming National Guard.Continue Reading
